Students of the University of Benin, UNIBEN who wish to replace identity card for any of the reasons mentioned above should present the following items:
1. Portal Login Details (username and password) or a Comprehensive Recent Online Printout
2. N1000:00 receipt from UNIBEN Bursary for ID Card, Holder and Lanyard.
3. Old ID Card (This applies only in case of defaced, broken, expired or outdated ID Card)
4. Security Report on the Loss of the ID Card (This applies only in case of missing ID Card)
5. Court Affidavit on the Loss of Identity Card (This applies only in case of missing ID Card) NOTE: Check your timetable to know the time allotted for your category as you will not be attended to outside then.
