University of Ibadan Medical Screening Procedure 2014/2015

Newly admitted students of the University of Ibadan for the 2014/2015 Academic Session should note the procedures involved with the yearly students screening exercise and adhere strictly to them.

These include:
1. Download Student Medical Examination Form from the Internet and complete it.

2. Chest Xray with a Radiologist's report duly stamped and signed by a Radiologist MUST be done before the commencement of the screening exercise from the approved X-ray Centres.

3. Report at the approved Screening Centres for Screening and Registration with the completed medical examination form, Chest Xray with a radiologist's report.

B) COMMENCEMENT OF SCREENING STUDENTS MEDICAL SCREENING FOR THE 2014/2015 SESSION.

The 2014/2015 Students Medical Screening for the newly admitted students of the University of Ibadan will tentatively run from 23rd March 2015 and end 3rd of April 2015 from 9am to 2pm daily.

The new intakes are to have their Chest X-rays done at the APPROVED X-RAY CENTRES before the commencement of the screening exercise.

They are to report at the DESIGNATED VENUES for screening with their Chest X-rays and the Student Medical Examination Form which has been downloaded from the Internet.

The designated venues for the screening exercise are: VENUES FACULTY University Health Service, University of Agriculture and Forestry Ibadan Basic Medical Sciences Pharmacy Veterinary Medicine New Sports Complex along Obafemi Awolowo Science Hall Technology Womens¡¯ Law Clinic, Faculty of Law Law Social Sciences Youth Friendly Centre along Technology Road Education Arts Students from Faculties of Agric, Basic Medical Sciences, Pharmacy and Veterinary Medicine to be screened at the University Health Service are to visit the ABADINA HEALTH POST beside the Diamond FM Station first for their height, weight and blood pressure measurements before presenting at the University Health Service for review by the doctors and data capture. A Mop-up Screening Exercise would be on these Saturdays (April 4th, April 11th and April 18th) at the University Health Service from 9am to 2pm for students who missed the main screening exercise.

C) FLOW-CHART OF ACTIVITIES AT THE SCREENING VENUES

1. Prospective students are expected to conduct themselves in an orderly manner and must be punctual for the screening exercise which commences at 9am and ends by 2pm daily.

2. Activities at the Screening Venues include:
Step 1:Arrival and obtaining a tally/number from the Social Workers
Step 2: Height and Weight measurement by the Social Workers
Step 3: Visual Acuity Test to check the eyes at the Optometry (Eye) Section.
Step 4: Blood Pressure check with the Nurses.
Step 5: Urine test i.e Urinalysis at the Laboratory section
Step 6: Reviews of Medical Examination Form, Chest Xray with a Radiologist's report and results of activities done from Steps 1-4 by the Doctors.
Step 7: Registration at the Records Section with a photocopy of the medical examination form.
Step 8: Data capture at the ICT Section with a photocopy of the medical examination form.
Step 9: Completion of Screening Exercise and Departure.

D) APPROVED X-RAY CENTRES FOR THE 2014/2015 STUDENTS MEDICAL SCREENING Newly admitted students of the University of Ibadan for the 2014/2015 Academic Session are enjoined to have their Chest X-rays done at these designated X-ray Centres before the commencement of the screening exercise.
All Chest X-rays MUST be accompanied with a RADIOLOGIST'S REPORT duly stamped and signed by a RADIOLOGIST.
The designated X-ray Centres are: 1 2 3 4 X-RAY CENTRES Komis X-ray Centre, Total Garden Area, Ibadan beside NMA House Metro X-ray Centre, 2nd building beside Alleluiah Petrol Station just beside Sango Garage University College Hospital (UCH), Ibadan Ultrarad X-ray Centre, University Health Service, University of Ibadan

E). DESIGNATED VENUES FOR THE SCREENING EXERCISE

Notes: students from faculties of Agric & Forestry, Pharmacy, Basic Medical Sciences, Veterinary Medicine for screening at the university health service are to visit the Abadina Health Post beside the Diamond Fm Station first before presenting at the university health service for the completion of their screening.
