Usman Danfodiyo University Teaching Hospital, Sokoto hereby announces SALES OF ADMISSION FORMS into BASIC MIDWIFERY
ENTRY REQUIREMENT:
- Applicant Must Possess FIVE CREDITS In Not More Than Two (2) Sittings, in the following subjects: English Language, Mathematics, Physics, Chemistry and Biology from WAEC, NECO, AND NABTEB
PROCEDURES FOR APPLICATION
1. Interested candidates can obtain the application form online on payment of a NON REFUNDABLE sum of Seven thousand naira (N7000) only through remita.
2. Candidates should visit http://www.remita.net/ and select Usmanu Danfodiyo University Teaching Hospital Sokoto from the MDA (Ministry, Department and Agencies) Remita Payment menu. Select UDUTH School of Midwifery under the service then generate remita (RRR) Number of the sum of (N7000) Seven thousand naira only.
3. Receipt of remita payment should be presented to SOM-UDUTH CASHIER and collect cashier receipt. BEFORE OBTAINING YOUR SERIAL NUMBER AND PIN.
4. Visit https://www.spbm.uduth.org.ng/basic online, form 2.php and serial number given to you.
5. Fill your information accurately and submit after acknowledgment.
6. Please come along with a clear printed coloured copy of your application form on the date of entrance exam.
