Eligibility Requirements
Candidates applying for admission into Admission Programme must possess a minimum of five (5) ‘O’ level credit including English Language and Mathematics at not more than two (2) sittings in WASC, GCE, NECO or its equivalent. Other credits must be relevant to the Programme as contained in JAMB brochure. Furthermore, awaiting result is not accepted.

- Four (4) Passport Photographs.
HOW TO APPLY FOR SOPBN NEC ADMISSION FORM

Application form is obtainable online at www.eyecenter.gov.ng after paying a non-refundable fee of Ten Thousand Naira Only (N10,000) through remita using the process below:
APPLICATION GUIDELINES
Step 1. Log in to www.remita.net and click on pay TSA & States, then select FGN: Federal Government of Nigeria.
Step 2. Fill the remita billing form using the following format:
Step 3. Name of MDA/What do you want to pay: write National Eye Centre Kaduna.
Step 4. Name of Service / Purpose: Select Education Revolving Fund.
Step 5. Description: Write Post Basic Ophthalmic Nursing Form
Step 6. Amount to pay: write 10,000
Step 7. Payers name, phone number, and email: write Applicant full name, phone number and functional email address.
Step 7. Confirm all entries are accurately imputed as guided then submit.
Step 8. Print payment invoice and proceed to any commercial bank for payment.
FOR FILLING OF APPLICATION FORM:
Step 1. Scan recent passport photograph, dully stamped remita payment receipt from the bank.
Step 2. Log in to www.eyecenter.gov.ng
Step 3. On the Home tab, click on Education then select School of Post Basic Ophthalmic Nursing and fill the Application Form.
Step 4. Read the instructions carefully, fill the application form online and submit.
Step 5. Print out application summary.
Step 6. Bring along the printed application form, Original and (1) photocopy each of your credentials, Remita payment receipt and two (2) self addressed envelope to the school on the entrance examination day.
