1. Admission Letter (JAMB) from the net.
2. Admission Letter (KASU) from the net.
3. Clearance Form from the department or DAP.
4. Matriculation Oath Form.
5. Fees Form.
6. KASU payment Receipt(s).
7. Medical Report-MR.
8. Affidavit of any kind (Name, Age, Indigenization, Loss of Documents, Testimonials etc).
9. Student Information Form- SIF.
10. Course Registration Form- CRF.
11. JAMB Slip with picture.
12. Internet copy of WAEC and/or NECO.
13. Primary School Certificate.
14. Certificate of Indigenization.
15. Birth Certificate or Declaration of Age.
