The deadline for registration was earlier announced to be 12 December, 2016.
However, it seems registration is still being accepted from interested individuals.

b. Centre Address
c. Address State
d. Centre Representative Name
e. Email Address of Representative ([email protected])
f. Telephone number of Representative (eg. 0701234567890)
g. Centre Ownership Type (e.g Private Owner, Public Institution, Private Institution)
h. Centre Capacity
