Please further to our directive on the downloading, filling, signing, and stamping of the students undertaking form to be of good conduct, and on the prevention of the spread of covid-19; students are hereby directed to now submit their downloaded, filled, signed, and stamped undertaking form to their departmental HoDs who will forward same to their respective Deans for onward Submission to the office of the Dean, Division of Student Affairs.
This is to ensure that crowding at the Office of the Dean, Division of Student Affairs is discouraged as a safety measure to help prevent the spread of COVID-19.
The Dean of the Division of Student Affairs Will forwards the collected forms to Deputy Registrars of Records and statistics who would place the forms in the personal files of the respective students.
Please all Deans and HODs should ensure compliance to this directive,
