To apply for the form you must have met the following requirements;
- Must obtained at least five (5) Credits Passes at GCE O/L, SSCE, NECO, or their equivalent. The subjects must include English Language and Mathematics as well as three other relevant subjects

STEP 1: Proceed to Federal Medical Center Asaba to make a non-refundable fee of Fifteen Thousand Naira (15,000) Only. For online applications accepting.
